The Fanatics Bariatric Surgery Expense Reimbursement Program is designed to provide financial support to eligible employees who require medically necessary bariatric surgery.

Who’s eligible?

All benefits-eligible US employees* are eligible for this program after completing one year of employment. Time spent as a temporary, part-time, or seasonal employee is counted toward this requirement.

*Benefits described in this site may not be available to employees covered under a collective bargaining agreement. If you have questions related to your collective bargaining agreement, please contact your HR Business Partner.

New policy effective July 1, 2023

As of July 1, 2023, the bariatric surgery program will be accessible to employees through their Fanatics medical and pharmacy coverage, rather than through a reimbursement program. This approach will allow access to high quality care, as well as clinical treatment and guidance to support your medically necessary needs at a negotiated rate while improving your overall well-being. Coverage will only be available to employees enrolled in a Fanatics medical plan. The current method of reimbursement is still being used until June 30, 2023.

Flexible benefit payments

that can be made in either a lump sum or in smaller amounts as expenses are incurred.

Note: Reimbursement payments

are considered taxable income by the IRS. The amount of your reimbursement payment is increased to cover taxes.

Request reimbursement using the Bariatric Surgery Expense Reimbursement Request Form and provide appropriate supplementary documentation (e.g., you must include both invoices and corresponding proof of payment).
